How to Fill Out Registration Cards-Procedures for Piedmont Staff:
  1. Give parent the proper color-coded registration card for student's rising grade level.
  2. Have parent fill out top half of card-contact info,etc
  3. Check and assist (if parent will travel they should add a summer contact number, circle medical needs (allergies, etc) "Special concerns"=add any other information parent would like teachers or counselors to kow , emergency contact is someone other than themselves)
  4.  Select course choices (year or semester) Note level placement is by CMS rubric upon receipt of EOG scores. Honors level is usually 95%ile and above.
  5. Have parent and student sign (if stundent not present, parent print student name in signature space
  6.  Date and initial the form
  7. Turn form over. Read together and sign the back. Point out community service hours, transportation, dress code, internet access, IB terminology and program, summer reading and math assignment
  8. Remind parents all info is on the website and to check it regularly (they can make a calendar reminder or do it weekly. Summer reading assignments will be posted by last day of school, Parent 101 meeting dates and open house dates will be posted in August)
  9. Give parent the Powerpoint / FAQ sheet to keep.  Answer  questions.
  10. Return completed form to Ms. Stone 

Things to note:

Do not choose semester electives if student chooses Band or orchestra. X out that box. 
IF student is not in band or orchestra, X out that side of the box and rank elective options 1-4, 6th graders can do a second language if desired in the semester course options. 
7th graders and 8th graders must keep same language they chose in 6th.
